Frontal-Lateral Tongue Cancer – Complex Treatment and Results

Abstract

Introduction. Oral cavity carcinoma is the sixth most common cancer in the world, with over 300,000 new cases diagnosed every year. The causes and frequency of oral cavity carcinoma are related to various factors which may be genetic (oncogene DNA mutation), environmental (physical and chemical factors) or due to lifestyle (tobacco use, alcohol, diet, occupational exposure to harmful agents). Tongue carcinoma is among the most common malignant tumors of the oral cavity.
Materials and methods. Our study includes 43 patients, diagnosed and treated at the Otorhinolaryngology Clinic of the University Hospital „Sv. Georgi“ EAD – Plovdiv, over a period of 8 years (2009 – 2016) – 39 male and 4 female. The distribution according to stage is as follows: Stage I, 1 patient – 2.3 %; Stage II, 8 patients – 18.6 %; Stage III, 10 patients – 23.2%; Stage IV, 21 patients – 48.8%; recurrences, 3 patients – 6.9%. Seven of the patients were treated using hemiglossectomy with labiotomy and mandibulotomy. Tracheostomy was performed on all of them and a nasopharyngeal airway was placed. These were all stage III and IV patients.
Results. After the multimodal treatment of patients with front-lateral tongue carcinoma by hemiglossectomy with labiotomy and mandibulotomy,
and postoperative radiation treatment, the following results were achieved: survival up to 4 years – 1 patient, up to 3 years – 1 patient, up to 1 year – 2 patients.
Conclusions:
• Hemiglossectomy with labiotomy and mandibulotomy in front-lateral
tongue carcinoma patients provides good visual tumor control and enables its radical removal.

• The diagnosis of front-lateral tongue carcinoma needs to be early in order to achieve better therapeutic results.

• Early diagnosis and small tumor volume enable the preservation of a larger part of the tongue and in particular its tip following radical excision, which is important for articulation and speech intelligibility.

• The multimodal treatment of patients with front-lateral tongue carcinoma leads to improved results compared to single modality surgery or radiation therapy.
