Authors’ Modification of Transparent Trocars for Endoscopic Surgery – rate of evacuation in 58 spontaneous intracerebral haematomas

Abstract

Introduction. Surgical treatment of ICH is still subject of controversy regarding the indications, timing and methodology
Materials and methods. The study was conducted with clearly defined criteria based on thorough analysis of the literature. By craniectomia punctiformis and a modification of the authors of transparent trocars with dimensions: length– 80 mm, width – 11 mm and beveled tip with angle of 45 degrees is performed endoscope-assisted evacuation of ICH in 58 patients divided in 4 groups. The patients were followedup clinically and radiologically and examine the rate of evacuation of hematoma.
Result. Timing of the procedure was within 48 hours of clinical manifestations. Rate of evacuation of ICH: 95% subcortical group, 90% putaminal group, 35% thalamic group and 75% cerebellar group. Mean operative time was 59 minutes. Results according to Glasgow Outcome Scale Extended (GOSE) were evaluated 1 and 3 months after intervention.
Conclusion: The data show that the rate of evacuation in spontaneous intracerebral hematomas with modified system of transparent trocars for endoscopic surgery is an effective treatment option in selected patients.
