Ранжиране на риска, свързан с факторите, влияещи на преживяемостта на оперираните със синхронни чернодробни метастази от колоректален карцином

Abstract

Проучването обхваща 101 оперирани пациенти със синхронни чернодробни метастази от колоректален рак, 63% от които първично резектабилни. Установи се че полът, възрастта, локализацията на първичния тумор, размерът на тумора, Т стадият и G стадият не оказват статистически значимо влияние на преживяемостта. Значимо влияние имат: броят на чернодробните метастази, техния размер и разпределение, лимфоваскуларната инвазия и кръвозагубата. Оперираните, които не са носители на нито един от тези фактори имат 3 и 5 годишна преживяемост съответно 83% и 50%, тези с 1-2 фактора - 40% и 35%, а 14% от носителите на 3 и повече от тези фактори преживяват 3 години. Разликите между показателите в отделните групи са значими. Установяват се значими разлики в преживяемостта в зависимост от вида на хирургичната резекция - голяма или малка с носителство на различен брой от посочените фактори. Мнофакторният Кокс анализ потвърждава значението най-често на разпределението на чернодробните метастази и техния брой, наред с вида на чернодробната резекция. Не се намира значима разлика в преживяемостта при оперираните със синхронни и метахронни метастази. Резултатите от проучването могат да бъдат полезни за практиката, при вземане на решение за вида на хирургичната интервенция и последващото лечение
