Comparative analysis of disease activity assessment scales in juvenile idiopathic arthritis

Veronika Zhelezova-Mineva, Boryana Varbanova

Abstract

Introduction: Juvenile idiopathic arthritis (JIA) is a chronic inflammatory disease with a heterogeneous nature. Over the past 2 decades, a number of scales have been introduced to assess the activity of different forms of JIA.

Aim: The main objective of the study is the application and comparative analysis of the most commonly used scales in practice for assessing activity in patients with different forms of JIA.

Materials and Methods: We have conducted a retrospective study, including 40 patients up to 16 years of age with different forms of JIA, different duration and degree of disease activity, undergoing different basic treatment. Patients were divided into three groups according to the type of pathology: enthesitis-related arthritis (ERA), oligoarticular JIA, and polyarticular JIA. Disease activity in all patients was assessed with three scales for complex assessment: JADAS-27, cJADAS, and JSpADA.

Results: The results of the comparative analysis show similarity in the assessment of activity measured by the different scales in the three groups of patients (p = 0.01)

Discussion: JADAS-27 and cJADAS scales are widely applicable in patients with JIA. JSpADA is a scale designed to reflect specific characteristics of one of the categories of JIA-ERA. The comparative analysis of the results of the study shows that JADAS-27 and cJADAS scales have similar reliability to the JSpADA scale in all three groups of patients.

Conclusion: The study demonstrates that, although non-specific, JADAS-27 and cJADAS scales can be used to assess activity in patients with ERA.
