The adamantinoma is а tumor which is mеt with comparatively rarely-0,04 per сеnt of all necropsy specimens investigated.
The histological picture of the adamantinoma is presented chiefly bу nidi (resembling а developmental period of the enamel organ), composed periphericaly bу columnar cells with clear protoplasma, whereas their nuclei are situated in the interior. Still mоrе internally localized ist the zone of the polyhedral аnd stellate cells, which occasionally show dystrophic changes, flat-cellular metaplasias, as well as formation of cysts. Depression in onе or more points of the nests are noted, similar to the еnаmеl orgarn during the process of formation of the dental сар. Along the margins of the aponeuroses are observed mainly cylindric cells, whereas situated more internally the zone of the polylhedral cells is disclosed. lt is stated that very often the aponeuroses consist опlу of cylindrical cells, which are· thin аnd strongly infiltrate the stroma.
ln single cases higlily differentiated cellular tlements are found - ameloblasts, odontobasts, as well as occurrence of preenamel, predeпtin (dentinoid) аnd „enamel pearls".
Among the patients followed up over а longer period of time, during which several biopsy studies have bееn carried out, every consecutive biopsy discloses differentiation of cellular elements аnd а pattern similar to that of the basiloma.
The stroma of the adamantinoma is presented bу connective tissue displayng various degrees maturity, with different quantity bооd vessels, hеmorrhages аnd hernosiderin; occasionally giant-cells are found of the osteoclast type. lп single cases lyrnphocytic infiltrates are observed in the strorna,which can bе interpreted as strornatic reaction. The affecled bone tissue shows dystrophic changes manifested in various extent. The strorna reaction and the bопе changes are syrnptorns indicating the rnalignant developrnent of the adamantinorna.
The histological picture of the adarnantinorna shows а great variability. Depending in the degree of rnaturity of the parenchyrna, typical adarnantinornas are differentiated -- cornposed of epithelial formations characteristic for the various periods of developrnent of the enamel organ, and atypical adarnantinornas - represented mainly bу infiltrating aponeuroses, displaying the pattern either of basilorna or some olher malignant turnor.
As indicated, ап adarnantinorna is basically а benign turnor, which in the lapse of its developrnental period accounts for widespread lосаl infiltrations, recurrences аnd eventually even metastases.
