The рroblеm of the diagnostic value ot the rheumatoid factoг tess (latex test and the heterohemoagglutination test of Waalere-Rose) in rheumatic polyarthritis is fairly well elucidated in the literature. Тhе prevailing view claims thе latex test to be as а rule negative оr it appears positive in the same percentage as it is in healthy individuals. The values of the Waalere-Rose test аrе higher: 15,6% (1);5-10% (12); 10% (4); 3% (after 3,5) etc. as far as the combined articular and cardiac form оf rheumatic fever without circulatory failure is concerned. What is actually the situation wilh the presence оf the rheumatoid factor in the chronic phase of rheumatic fever, i.e. in chronic rheumatic valvular heart disease - this is а prblem scarcely discussed in the literature. А more systematic study of this рroblеm is carried out bу V. Tsonchev аnd Т. Pilosof, who establish positive Waalere-Rose test in 29% of the cases with rheumatic valvular heaгt disease in the slage of advanced congestive heart failure. They assume this positiyation to bе nonspecific and ought to bе ascribed to dysglobulinemia associated with dystrophic changes which occur in the liver under the influence of congestive heaгt failure.
Since positive tests may occur in chronic rheumatic valvular heart disease, particularly in the stage of congestive heart failure (chronic proliferative inflammation, destructive tissue changes, the ocourrence of autoimmune processes and of dysproteinemia) we set ourselves the task to innestigate the level of the rheumatoid factor in these cases, employing simultaneously the latex and the Waalere-Rose tests in patieпts suffering from different degrees of congestive heart failure.
