The author, who is the promoter in Bulgaria of the doctrine for the surgical approach and·the rational surgical access, further extends its theoretic and practical elaboration, submitting а new definition of the idea for operative approach, including all the elements of it performance. Не adds а new, third criterium in the determination of the idea for rational operative access, namely: the direction of the incision (the other two elements being the localization and the size of the incision).
