The EU is focusing its efforts on high quality community care. Тhe benefits of the knowledge, shared between the EU Member States have recently been taken into account to better address the challenges of this process. Integrated healthcare is defined differently across EU countries.
The main purpose of the report is to assess properly the factors that require integration of care, focusing on the prospects for a comprehensive treatment plan, information sharing, communication and collaboration.
