The differential diagnosis between primary open-angle glaucoma and non-glaucomatous optic neuropathies represents a significant challenge in ophthalmic practice. The presence of morphological changes in the optic disc often initially raises suspicion of glaucoma.
This article presents two clinical cases of patients with glaucoma-like structural changes, in whom the clinical presentation and medical history raise the possibility of a non-glaucomatous, potentially post-traumatic etiology. In both cases, the atypical course of the disease is particularly noteworthy, suggesting from the outset the need to consider alternative diagnoses beyond the conventional diagnostic framework.
The presented cases highlight the importance of a comprehensive assessment of structural and functional parameters, thorough medical history taking, and a targeted differential diagnostic approach.
