Abstract

For the first time in this country reference is made to a case with perforation of the aorta, caused by Silmonella strain. The disease runs a course characterized by prolonged febrility (up to several months), abdominal pains, sacralgia and absence of medial and inferior abdominal reflexes. The palpatory examination of the supra-umbilical region reveals a painful compact mass, the size of a hen-egg, which attains weak pulsations one day prior to the prolapse into the duodenum, followed by instantaneous death of the patient. Salm. choleraesuis var. Kunzendorf is isolated from the blood. At autopsy, rupture of the aorta is established 4 cm above its bifurcation, with the adjacent pseudoaneurysm cavity filled up with blood. This cavity is communicating with the posterior wall of pars ascendens duodeni. At the site of the prolapse, a heavy atheromatosis is also found. The rupture is assumed as the result of interaction between two factors: atheromatosis of the aortic wall with the added effect of Salmonella fpctor. The antibiotic treatment with chloramphenicol and streptomycin applied, accounted for the subsiding of the febrile condition, but did not prevent the rupture of the pseudoaneurysm into the duodenum. Salmonella arteriitis and ruptures, reported in pertinent literature are discussed.
